August 8, 2014
Dysart Reporter

Back to school registration will be held on Monday August 11th and Tuesday August 12th. You will be able to register between 1 pm and 7 pm both days. Please register your students at the building which they will be attending classes.

If your child will be a new student at the middle school or high school, registration will be held August 13th from 9 am to 12 pm. It is very important for students and parents who are new to this district to attend registration at this time because they need to sign an authorization release form from the students previous school to receive their records. At this time they will also be able to take names and grades to assign your children to the correct classes and bus routes.

At registration parents will be asked to fill out or update the student's enrollment card. This card contains the following information, so please be sure to bring this all with you; (1.) Information on where to locate parents in case of illness or injury. (2.) Name of physician and hospital choice. (3.) Phone numbers of relatives and friends or neighbors who may be contacted if the child's parents cannot be reached. This is preferably someone who can be reached at all times, has transportation, and lives near you and your children.

Some of the forms you will be required to completed are able to be located at the district's website WWW.union.k12.ia.us. These forms are listed on the website by building and you may print them off for each student you are enrolling and have them filled out before you come to register. A few forms are also available to be filled out on-line.

All student fees are due and payable at the time of registration. All checks must be made out to Union CSD. Please have the following ready when you come to register; ALL students will need emergency contact information, registration fees, and the Internet Acceptable Use Policy form. For kindergarten and New Students a birth certificate and Health records and any information that was given during Kindergarten Maturity Week. If applicable you will also need to bring free/reduced price meals application, the standard fee waiver application and the medication permission form.

There will be orientation at the Middle School on August 19th from 5-6 pm. This will be for all students who are new to the district and 6th graders who will be new to the school. For students and parents in La Porte who will need a ride, you can catch a shuttle in front of the high school at 4:30 pm.

The High school orientation will be August 19th at 6:30 pm. Students from Dysart who need a ride will be able to catch a shuttle bus at the middle school at 6 pm. This will be for all 9th graders who are new to the school, and all children who are new to the district.

D-G and LPC Elementary open houses will be held August 18th both from 6 to 7 pm. All students are welcome at this, as most of them are excited to see their new classrooms and teachers, so stop on by to get some information on the up coming year!

The school year will begin on August 20th, 2014. The first bell will be at 8:19 am at Union High school, 8:20 am. at the Middle School and both Elementary schools. Office hours for all buildings will be 7:30 am. to 3:30 pm. The first set of parent teacher conferences will be held August 27th and 28th from 3 to 8 pm. All students will be released from school at 12 noon on both of these days, and there will be no school on Friday August 29th. Please plan with your children ahead of time for all early dismissals. It may be difficult to get through to the school on these days, so it is important that you and your child have a plan set up for these days.

Please be sure to keep your eyes out for the Union Community School District Newsletters throughout the year.
